Employee Vax Incentive has 3 more weekly prize drawings, gift cards for all who enter, & grand prize drawing on Dec. 20
Employees who have not yet filled out the eForm for the Vaxination Incentive program have 3 more weeks to enter and receive a $50 gift card just for turning in the form.
Prize drawings for weekly gifts will take place today, and on Dec. 13 and Dec. 20. In addition, all fully-vaccinated employees who turn in the eForm are entered in the $5,000 grand prize drawing that will take place Dec. 20.
- Prizes slated for upcoming drawings include a Queen City “Night Out” gift card ($250 value) and a Planet Fitness annual membership ($340 value).
- When you turn in the Employee Vaccination Incentive eForm and provide proof that you’re already fully vaccinated, you’ll receive a $50 gift card.
- When you turn in the eForm and provide proof that you’ve obtained (since Nov. 22) your first shot of a 2-dose vaccine, or completed a 1-dose vaccine, you’ll receive a $100 gift card.
As of Dec. 3, nearly 260 College employees have entered the Incentive Program and three first-time vaccinations have been reported.
