Employee Vax Incentive has 1 week until grand prize drawing

Employees who have not yet filled out the eForm for the Vaccination Incentive program have until 5 p.m. on Friday, Dec. 17 to enter.

Fully-vaccinated employees who turn in the eForm are entered in the $5,000 grand prize drawing that will take place Dec. 20.

Prize drawings for weekly gifts will take place today and on Dec. 20.

• When you turn in the Employee Vaccination Incentive eForm and provide proof that you’re already fully vaccinated, you’ll receive a $50 gift card.
   
• When you turn in the eForm and provide proof that you’ve obtained (since Nov. 22) your first shot of a 2-dose vaccine, or completed a 1-dose vaccine, you’ll receive a $100 gift card.
