Student Vax Incentive e-Form is easier to use; Week 5 prize winners announced

Students who want to be part of the College’s Vaccination Incentive Program now can access the program entry eForm without logging in to Blackboard/MyServices.

The new eForm link is https://web3.cincinnatistate.edu/eforms/eform.aspx?form_id=2033.

• Faculty and staff members are encouraged to share the link with students who have not yet entered the incentive program, and let them know about opportunities to win weekly cash prizes and the $10,000 grand prize, which will be awarded on Dec. 15.
   
• More than 950 students have submitted the eForm indicating they are fully vaccinated, and more than 40 students have reported receiving their first COVID-19 vaccine during the five weeks since the incentive program started.
   
• More than 50 employees and students were vaccinated at the on-campus clinic on Nov. 4, and more than 40 flu shots were given at the clinic.

Associate of Arts major Beatrice Crockett is the fifth winner of a $1,000 prize in the Student Vaccination Incentive Program.

Beatrice received her check last week from Student Government Association President Shane Winslow.

Beatrice said, “In order to change, you have to be part of the change. No matter what it takes…self-discipline, courage, and gumption!”

Culinary Arts major Charlotte Huxel (left), winner of a $100 prize in the Week 5 drawing, said she is vaccinated “to stay healthy and keep those I love safe.”
